Durable Power of Attorney for Health Care

The Durable Power of Attorney for Health Care appoints someone (who is referred to as your Agent) who is able to make health care related decisions for you in the event that you are not able to make decisions. We feel this is one of the single most important documents you can have as it directs the medical care providers to know what you have expressed that you want done, or in some cases, don’t want to be done for you. Your Agent has the authority to consent to surgical procedures, develop a plan of care, review your medical records, and speak with your doctors. It is important to discuss your preferences and wishes in advance with your Agent so they are able to act in a manner similar to what you would do if you were able. Your Agent needs to be someone you trust and who will be not hesitate to act on your behalf to make necessary decisions. We recommend that you appoint a primary Agent as well as a secondary Agent in the event the primary Agent is unable to serve.
